What the visit request form collects
The form asks for your name, the phone number or email you choose to give us, how you prefer to be contacted, the general reason for your request, a preferred date, your language, and your consent to be contacted. That is all we need to call you back.
The form does not ask for symptoms, diagnoses, medications, insurance numbers, records, images, or payment details. Please do not type them into it. To send records, call the front desk first.
What happens to a request
Your request goes to the front desk at our Sunny Isles Beach office. If online delivery fails, the form tells you to call 786-320-5445 instead. We use your details to answer you and to arrange a visit.
Technical logs
We keep basic technical logs to prevent abuse.
Other services
Some ways of reaching us run on services that belong to other companies. Those companies handle your messages under their own privacy terms.
- WhatsApp. Messages you send us by WhatsApp pass through Meta’s systems. Please do not send clinical records or health details by WhatsApp. Our WhatsApp number: 305-900-7042.
- Instagram. Our Instagram page runs under Meta’s terms.

You can ask us to show you, correct, or delete the contact details you sent through this website.
Clinical records
This website is not a patient portal. Clinical records are handled through our clinical systems and our Notice of Privacy Practices.
